    Has anyone had UNC deny claims for the Clia number not being in Box 23 (prior authorization number)? We place it in the box that says CLIA: I think it is box 31. They have been paying it like that since the beginning and now denied it saying it needs to be in box 23. I am afraid we will change it and then they will deny because it isn’t in the CLia box. I know we could put it in both but it really is a lot to keep up with who wants it where! Ugh! Just wanted to know if anyone else has had this issue?

    There was a bulletin that came out a month or so ago from UHC saying that that was now a requirement.

    If the ClIA number that UHC has is expired; they will deny it. You need to send them your new CLia #.

    It also says when you do this you have to put Referring Provider in Box 17 I think.
