Healthy Blue Hemoglobin

I have just started getting denials on hemoglobin with the diagnosis code 85018. I know it’s not our CLIA number and this is the code I have always used with no modifier for Healthy Blue. Any ideas why? Thanks

Maybe they have started requiring modif1er QW like the other 2 plans

Healthy Blue has never paid me for a hemoglobin..

I tried that and still got denied. Also tried TC modifier that Medicaid requires and it was denied also.

85018 with no modifier N822missing procedure modifier N823 Incomplete/invalid procedure modifier
85018 with QW modifier The procedure code is inconsistent with the modifier used, this should be billed with the appropriate code for these services.
85018 with TC modifier The procedure cods is inconsistent with the modifier used

You do have to use QW for the modifier now.

Do you still use a 85018 and Z130 diagnosis code? Sorry for so many questions, I have tried it every way imaginable and can’t get it paid. Never had trouble before.

I used to get paid all the time, too. Now I have filed it and was denied. Then I filed it under z13.0, 85018 QW and was denied. What’s the deal????
