    We have been asked by local daycares to sign off as the
    Health Care Provider” on the Tuberculosis (TB) Risk Assessment Form (MO 580-3015). Our issue is that there is no clear cut guidance on what a Health Care Provider is – basically, can an RN sign it? We do not have a physician in-house, but operate under standing orders. Our regional TB nurse has said we can, as per Childcare Licensing, as long as there are no TB risk factors acknowledged on the form.

    Are your RN’s signing the TB Risk Assessment form for childcare licensing, and if so, do you have a physician in-house who is involved?
    If so, what protocol do you have in place, and would you be willing to share your policy and protocol?
    Do you charge a fee for this service, and if so, how much do you charge?

    We do not sign the TB Risk Assessment form; we will put TB skin testing results on the form and sign by the results. As an RN do not feel it is appropriate to sign a form that asks for health care provider’s signature. Ruth Bock, Gasconade County
