We are also getting denials for hemoglobins for the following reasons: invalid clia number or No Authorization on file that matches services billed. I spoke to Laina Tobias at Home State yesterday regarding these issues. She said for Clia waived procedures such as the Hemoglobins, you should put the Clia number and a QW modifier after the procedure code. According to Laina, you should not get denials if you do this.
