for medicaid and managed medicaid We always bill 99211, which doesnt require a Dr thats why they allow health dept to use it with vaccines and they have to do height, weight, and education which im sure you already do the hand outs and such. We bill it under Z00129 for 99211 and Z23 for vaccines. I have never had an issue with paying. I use the Z02.89 for WIC office visits.
If you bill a 99212 you must have the following and it is for the evaluation and management of an
established patient, and requires at least two of these three key components be present in the medical record:
1. A problem focused history
2. A problem focused examination;
3. Straightforward medical decision making
A tip for billing 99212 is that the presenting problems are usually self-limited or minor.
Typically, 10 minutes are spent face-to-face with the patient and/or family.
