in the CPT 2018 standard code book it says–
99211 Office or other outpatient visit for the evaluation and management of an established patient, that may not require the presence of a physician or other qualified health care professional. Usually the presenting problems are minimal. Typically, 5 minutes are spent performing or supervising these services.
You do not have to have a physician and you do not have to be a nurse to perform services and bill for this code. You can be a medical or nurse assistant, WIC HPA, WIC certifier, nutritionist.
